COUNTDOWN CONTESTANT APPLICATION FORM

You are applying to be considered as a contestant in the programme entitled, “Countdown” (the “Programme”), which ITV Studios Limited trading as Lifted Entertainment (a company registered in England and Wales) with its registered office at ITV White City, White City Place, 201 Wood Lane, London, W12 7RU (registered company number - 0310625) and with its place of business at Orange Tower, MediaCityUK, Salford, M50 2HE (“we”, “us”, “our”), intends, but does not undertake, to produce for initial transmission by Channel Four Television Corporation (the “Broadcaster”).

The Programme is a quiz programme in which individuals compete to solve linguistic and mathematical puzzles.

Please ensure you meet the Programme’s eligibility requirements set out below (the “Eligibility Requirement(s)”) and read the Programme’s application terms and conditions here (the “Terms and Conditions”) before you start completing the application form (this “Application Form”). You must fill out the ENTIRE Application Form. If any question is not applicable to you, please type “N/A” in the space provided.

ELIGIBILITY REQUIREMENTS

1. You must be aged at least 16 (sixteen) years of age due to the scheduling requirements of the Programme and the studio environment in which the Programme is recorded.
2. You must be legally resident in the United Kingdom or Republic of Ireland and have a current address within the United Kingdom or Republic of Ireland.
3. You are not:
   1. currently employed by us or the Broadcaster and have not been previously employed by us, any of the ITV plc group of companies, or the Broadcaster; or
   2. a live-in partner or immediate relative (for example, mother, father, son, daughter, brother or sister) of an employee of ours, or the Broadcaster, or any of the ITV group of companies.
4. Unless otherwise permitted by us at our sole discretion, you must not have appeared in the televised stages of the Programme on any previous occasion.
5. We have the right at our sole discretion to add to, delete from, modify or amend these Eligibility Requirements and to select contestants for the Programme. In all circumstances concerning the interpretation and application of these Eligibility Requirements, our decision will be final.

YOUR PRIVACY

Here at Countdown, your personal information is really important to us. So important, in fact, that we have two privacy notices.

Firstly, any personal information provided in the Application Form will be collected and processed in accordance with our privacy notice (click here to view) (our “Privacy Notice”).

Secondly, we have a sensitive information privacy notice (our “Sensitive Information Privacy Notice”), which deals with personal information which is more sensitive than ‘everyday’ personal information, such as information about your health and criminal history (if any) (“Sensitive Information”). Sensitive Information will be collected and processed in accordance with our Sensitive Information Privacy Notice (click here to view). You can only submit an Application Form for the Programme if you consent to us processing Sensitive Information in accordance with the Sensitive Information Privacy Notice by ticking the box below to agree to this.

If you do not want to provide consent, want to withdraw consent or have any questions about how this information will be used, please contact us at myprivacy@itv.com. Where relevant, you will need to write separately to any third parties to withdraw consent from them. Please note, however, that not providing consent, or withdrawing your consent may potentially have an impact on your participation in the Programme and, depending on the circumstances, we may still be able to use your sensitive information despite your withdrawal of consent, for example, where we believe such use to be in the public interest, including for journalistic or artistic purposes.

This section is about your preferences for us to retain your name and contact details to keep in touch with you for future series of this Programme and/or other shows produced by any companies that operate within our family of companies. Whatever your decision here, please be assured it won’t influence our selection process for this Programme. Please note that if we do get in touch with you in the future, you may be asked to update us with any changes to the information you have provided and, if appropriate, submit a new application form.
