We are looking for fun and outgoing contestants to take part in an exciting, competitive crazy golf game show for ITV!

PLEASE READ THIS APPLICATION FORM, INCLUDING THE TERMS AND CONDITIONS, VERY CAREFULLY BEFORE COMPLETING AND SUBMITTING YOUR APPLICATION. 

Please use this form to apply to take part in a gameshow television programme, provisionally entitled, “NEW PRIME TIME CRAZY GOLF GAMESHOW” (the “Programme”). The Programme will be produced by FremantleMedia Limited trading as Talkback Thames and Mitre Studios Ltd (together, the “Producer”, “us”, “we”, “our”).

Before continuing, please read and confirm that you acknowledge and agree to comply with the following:

1. Please note that initial auditions will be carried out via video link. If further auditions are required, these may be held in person (subject to change). Further filming may be held in London and/or a location outside of the UK. All locations are to be confirmed and are subject to change.

2.     All applicants acknowledge that if their application progresses and they are selected to appear as a contestant on the Programme, the applicant shall be required to comply with the Producer’s health and safety rules, guidance and protocols and health and safety policies and procedures and any specific production protocols, as such rules, guidance and protocols are updated and notified to you by the Producer from time to time.

Please note that the application process is strictly confidential and by completing this application form, you agree that you shall not, without the Producer’s prior written consent, disclose any information in relation to the casting process or any other matter concerning and/or relating to the Programme, including, without limitation, the premise and concept of the Programme.
